Case Management

Our experienced and caring case managers are all registered health professionals (nurses, social workers, occupational therapists) who have the background medical and mental health knowledge to address client needs proactively. Swanson & Associates’ case managers draw on an extensive network of community resources developed over years of experience, enabling them to facilitate cost-effective and accountable delivery of care and a treatment team that meets the client’s needs. File coordination is individualized, using a holistic, client centered approach as the client reintegrates into his/her life roles.
