Our team includes occupational therapists, physiotherapists, speech language pathologists, social workers and rehabilitation aides.
What you’ll do:
- Assess client needs for a wide range of diagnoses
- Coordinate rehabilitation and support services and facilitate the rehabilitation process
- Provide client, family and caregiver education, counselling and support in-clinic, or in-home
- Liaise with a range of third party stakeholders
- Facilitate effective communication with client, family, rehabilitation team and funding sources
- Write reports
- Utilize a holistic, client-centred approach
- Work with a strong interdisciplinary team of healthcare professionals
- Participate in peer mentoring groups to improve collective knowledge
What you’ll bring:
Required:
- A post-secondary degree in health care (e.g., nursing, social work, occupational therapy)
- Registered and in good standing with your respective regulatory college (required)
- Strong leadership and interpersonal skills
- Excellent time management, organizational and problem-solving skills
- Ability to work independently and within multidisciplinary team
- Excellent oral and written communication skills
- Strong critical thinking, decision making and analysis skills
- A drive for continued learning
- A valid driver’s license and a reliable vehicle
Preferred:
- Experience in rehabilitation
- At least 3 years of case management experience
- Strong knowledge of community resources
- Bilingual in French and English is an asset
- Other languages such as Arabic

- Skilled in the provision of a variety of therapeutic approaches including but not limited to Cognitive Behavioural Therapy, Mindfulness-based therapy and Strengths-based therapy

- Skilled in educating clients, family and support persons regarding the impact of trauma
