Discharge from the hospital marks a critical transitional period for the client. Numerous services may be required but often the client is unaware or unable to initiate the process. Both our case managers and our occupational therapists are experienced in supporting the client, liaising with the discharge facility and the client’s family, assessing requirements and implementing required recommendations. Assessment of attendant care needs can be completed early on so that the support services can be implemented at the time of discharge. Similarly, safety, mobility and accessibility equipment can be implemented as required.
